What about ctrl-/ for single-line (un)comment and ctrl-shit-/ for block (un)comment?

I quite like how Dreamweaver has a button in the gutter that allows selection of a lot of different types of comments. Single line, multiline, html, asp, etc. |

I don't think this is strictly necessary with PhpED since it's context-sensitive. That is, if you're in CSS, it'll use CSS comment syntax, but if you're in PHP, it'll use PHP syntax, and so on. I think all that's needed to address the OP's requests is access to the existing comment functionality from the context menu. |
