step 5 seems to describe what is happening. Unfortunately, the given workaround there does not seem to help.

All the symbols listed are expected to be defined in libphp4.so (or whatsoever php for apache module name is). Depending on the way you linked this module with apache, you may get different results.
I'd recomend you try pre-compiled Apache 2.0.XX for Mac OS X and php 4.3.11, 4.4.0 or 5.0.4 compiled yourself. The pair works good AFAIK.
