"Please wait -- Waiting for debug session" then "Failed to start debug session (timeout)"

I'm using a 3rd party web server, have verified and reverified that the correct debugger extension= line is in php.ini and the [debugger] section added.

It's showing established, and yet it will not connect.
Please help. I've had this working before, and I have run into this problem before, but I don't remember a specific fix. It was one of those things where it just started working and it didn't seem like I had changed anything.

What can I do for you?
Networking is very important but it's not all that make things going.
What's about the IDE and debugger and php versions you're using?
