Does PhpED PHP 5.4 keeps a log of any PHP startup issues.

If you start you start php from the Windows command prompt, it should display messages any startup issues (missing extensions, etc.)

I also think this NuSphere build of PHP displays messages both in a dialog as well as console output.

The SRV log is easy to get to. Whilst SRV is running it has an icon in the task tray, double-click that icon then in the window that pops up you can click on logs. However, I don't know if those logs will show PHP startup issues; I think it just shows requests, etc.
