Right-clicking on the tab for the selected file offers the Format Text With PolyStyle option. Can I set this up as a shortcut/button icon?

Yes, I see PolyStyle already setup under tools->settings->integration. Is it possible to get that "integration" associated with a button/icon on a toolbar?

yes, you can assign shorcut and you can add it to the tools menu. Regarding toolbar, it can't appear there directly, only as a copy of Tools>scripts dropdown.
First check "Show this command in the Tools menu", then optionally assign shortcut, then right click on the toolbar, select customize and drag tools->scripts with Ctrl held down into the toolbar.

Is it possible to assign shortcuts with prefixes to Integration-items as it is for the editor-shortcuts?
I'd like to have "Ctrl+K,D" (or "Ctrl+K, Ctrl+D") for Polystyle (as in VisualStudio).
