Don't know if this will be good enough for you, but I personally don't like a default file to run. So what I do is leave the field empty.
If you "run" with the default file empty it will use the file that is currently active in the editor. |

If you debug fileA.php and work/edit fileB.php that is somehow included, you'll find it useful to run/debug via Run->Repeat last run. Fill free to bind a shorcut for it.
