To set the "default file to run" you go Project, Project Properties & then set the "default file to run". Would it possible to add to the right click of open and closed files an option of "Set as default file to run". This would make it much easier to set/swap the default file during testing.

Don't know if this will be good enough for you, but I personally don't like a default file to run. So what I do is leave the field empty.
If you "run" with the default file empty it will use the file that is currently active in the editor.

If you debug fileA.php and work/edit fileB.php that is somehow included, you'll find it useful to run/debug via Run->Repeat last run. Fill free to bind a shorcut for it.
