After upgrading to build 4060 the cvs menu and icons have disappeared from the workspace tab.
I checked the Integration section and the settings are checked for 'Show this command in Workspace popup'
and both settings underneath are checked, 'for files' and 'for directories...', so it doesn't look like that has changed.

Check if Source Control is set to CVS in the Project Properties.

I checked. It is and the cvs menu does come up on the file tab, sorry should have given you that information too.

No, it comes up on the file tab, not in the workspace flyout.
